Summary

You are being asked to participate in the Radiation Oncology Registry. Participation in this study requires no additional obligations on your part; your participation means you are granting permission for your doctors to extract information from your medical chart and use this data for research purposes. Should any findings from this research be presented or published, your name, date of birth, or any other personal identifying information will not be included.

Description

Participation in the Radiation Oncology Registry allows your doctor to study your
medical chart for research purposes indefinitely. You may choose to withdraw your participation at any point. If you choose to do so, please notify our department in writing and we will stop using your medical chart for research. If your chart has already been used for research purposes, we cannot guarantee your removal from those data sets.
There is no maximum number of people that may participate in this study. Any person over the age of 18 who comes to our department for radiation treatment will be allowed to sign up for the Radiation Oncology Registry.
